Late Arrival

TREATMENT CANCELLATION POLICY
To ensure a smooth and enjoyable experience for all our clients, we have established a detailed policy regarding appointment bookings, cancellations, and arrival times. Understanding and adhering to these guidelines will help us maintain the high quality of care you deserve while allowing us to accommodate other patients effectively.
At the time of booking your appointment you will be asked to pay a non-refundable deposit that will be credited towards your treatment/s.

Cancellation:
Appointments are in high demand, and your early cancellation will give another person the opportunity to have access to timely care. This policy enables us to better utilize available appointments for our clients.
We understand that there may be times when you need to cancel or reschedule your appointment. Whenever possible, please provide 24 hours’ notice to make your appointment time available for other clients. If you must cancel on the day of your appointment, you need to call at least 2 hours before your appointment time. If you call less than 2 hours before your appointment, or if you do not show at all, this will be considered a "no show" and you will lose your non-refundable deposit and forfeit any discounts applied to the cancelled service. You may reschedule, but at your membership price however, two or more "no shows" in a row will result in a full charge for your treatment and you will need to pre-pay to schedule future treatments.
Emergency cancellations are assessed on a case-by-case basis and are determined at the therapist's discretion. Changes in Appointments: Just as you are asked to respect your massage therapist's time, your massage therapist will also respect your time. You can expect your therapist to be on time and ready for you at your scheduled appointment time. In the off chance that we cancel your appointment within 5 hours of your appointment time, you will receive an “enhancement” compensation rescheduled for the next appointment. (restrictions applied)

Collected Information
Your personal information is collected online through our contact forms only. This website currently has no applications or programs that gather information from your computer without your consent. Users of the contact form are required to give us contact information (e.g., your name and email address or phone number). This information is collected in order to verify your identity and for use in our records to contact you. Information collected at this website is kept in our records only as long as is necessary to provide requested services to our clients.
If you make a purchase on this website, you will be directed to the PayPal website, where they will collect the necessary information (e.g., name, address, phone number, email address) to process your payment and inform Healing Qi Oasis, LLC of your purchase. To purchase a gift certificate you may also need to provide the name, address and email address of the person for whom you are purchasing the gift certificate. This will be sent from PayPal to Healing Qi Oasis, LLC and will be used to process and ship your gift certificate. You will also need to provide PayPal with your credit card or PayPal account information for payment. Your credit card information is never provided to Healing Qi Oasis, LLC when you make a purchase through PayPal. Here is more information on PayPal's Privacy Policy.
At your first appointment, and at least annually, you will be asked to complete an intake form, which requests your demographic and health information. This form, and any discussion regarding its contents, will be kept confidential between you and your massage therapist. The intake form, any demographic data that is collected, and any subsequent notes related to your treatment sessions will be stored in a locked file, or in a password-protected computer file. This information will be used in the provision of services and in recordkeeping for the business.
Your Information
We will not give out any information you submit at our site or in the office to any third parties except in compliance with a request of a law enforcement agency. Under no circumstances will your name, e-mail, phone number, or address be sold to, or given to any other parties. We will never willfully sell, lease, or rent any of your personally identifiable information to any third party. You may occasionally receive email notifications from Healing Qi Oasis, LLC regarding updates and promotions. You will always have the option to "opt out" of receiving these messages.
Your massage sessions with your massage therapist are kept in the strictest of confidence. No information regarding your massage treatment session(s) will be released to anyone else, including your referring physician, without your written consent.

Professionalism/Therapeutic Relationship
Standards of Practice and Code of Ethics:
You can expect your massage therapist at Healing Qi Oasis, LLC to be professional and to be respectful of personal and professional boundaries at all times. As a Licensed Massage Therapist in the State of New York your therapist must abide by New York State law governing the practice of massage therapy, and by the AMTA Standards of Practice and the AMTA Code of Ethics. Your Therapist is a professional member in the American Massage Therapy Association (AMTA). Healing Qi Oasis, LLC does not discriminate on the basis of age, race, color, national origin, gender, sexual orientation, religion, socioeconomic status, political affiliation, or mental or physical disability.
Inappropriate Behavior
At Healing Qi Oasis, LLC you will receive a massage. It is against New York State law, and a Licensed Massage Therapist will never massage the genital area (male or female). Also, while work may be done on the muscles of the chest, a Licensed Massage Therapist will never work directly on the female breasts. Proper draping techniques will be used to protect your privacy and only the area of the body on which the therapist is working will be exposed. Sexually suggestive behavior will not be tolerated. The therapist may terminate the session at any time, at the therapists' discretion, if the client's behavior is deemed inappropriate. You will still be expected to pay for the session.
Grievances
Your therapist will make every effort to provide you with a customized massage that meets your needs, and assures your comfort and safety. Communication with your therapist is very important. Your therapist will expect you to let him/her know if you are uncomfortable in any way, so adjustments can be made in your treatment during your massage. The client has the right to refuse treatment and may terminate a session at any time.
If you have a complaint about your therapist, or Healing Qi Oasis, LLC please contact us so we may try to resolve your issues. If you are still unable to resolve the issue, or if you have a complaint of illegal or unethical behavior on the part of your therapist, or of Healing Qi Oasis, LLC you may contact the Licensing Board or professional organizations that oversee the practice of your massage therapist.
